•Evaluate patients and develops appropriate plan of treatment for diagnosis.
•Under a physicians order, provides evaluation and direct patient treatment in accordance with an established plan of care.
•Documents treatment according to company’s clinical documentation system to reflect skilled intervention.
•Communicates effectively with patients, families, caregivers, team members and supervisor.
•Completes assigned evaluations, treatments and screens timely as per company policy.
Requirements:
•Bachelor of Science, Master of Science Degree or Doctorate Degree in Physical Therapy and professional CT State licensure.
•Demonstrate ability to utilize independent and effective clinical judgement, problem-solving and decision making skills.
•Possess strong oral and written communication skills. Organizational and analytical skills required.
